So I was playing Xenoblade Chronicles (this game is expensive and rare now)
and I was fighting Metal Face. Of course, normal attacks can't exactly Face Mechon, so I kept doing 1 damage.

I forgot to use a chain attack so that my arts COULD inflict break, so that Reyn could inflict topple.

I have 2 I can think of right now. Once on Sonic Adventure, I was about to clear Tails' story, and I was against the Egg Walker (That part of the story's final boss). At the last hit, I deal the final blow, and then I get killed while it said I won. In other words, I had to reset the boss fight after winning.
The other one was in Mario Kart DS on the Rainbow Road. Apparently, there is a trick where you can "fall up" where you drive into a wall on a loop or corkscrew, and I failed the race. That means the Grand Prix had to be redone.

That kind of stuff happens way too many times in video games, and I cannot recall how many times I fell victim to these (although it wasn't in Sonic Adventure). It's what we call a Kaizo Trap. When I beat Wily Capsule for the first time in Mega Man IV (GB), I completely raged when the screen pushed me into a hole he had created, and that was just after I save stated, spoiling my otherwise perfect run.
